
I realize you don’t come here for healthcare advice, but since my wife manages 25,000 people’s plans and my mom and millions of older Americans just got word that Medicare Advantage is going up, I hear about it a lot.
I’ll give you the insider info. The numbers are not exact but give a good sense of where things stand. Over the past five years, the percentage of premiums received by insurers and paid out was in the 70% range for most insurers. That left them with about 30% for overhead and margins, which is pretty tight. But when you are talking billions of dollars, it is still not too bad. Last year those numbers crept up to the low 80s, making it unprofitable for most. In the companies that specialize in elderly care, those numbers actually reached 110% or more, meaning they are getting crushed.
In the past, Medicare has been free, and for many, the add-on package Medicare Advantage has also been free or very cheap, around $20 a month. The government can’t subsidize the increased expenses, so the insurers have to make up the difference, and that means raising monthly premiums. For most of America, which is on a fixed income, the extra $80 to $200 that is being discussed makes things tight. I know most of us would love to have insurance that cheap, but remember the last generation had much lower incomes and different expectations for cost. A big business to be in right now is helping people find health insurance. A concierge service that is paid a commission by the insurance companies. My sister-in-law retired to do that for a new career and is doing great. It requires a license, but it is not that hard. Older people love it, its free, and the customer satisfaction rate is 10X of them trying to do it over the phone blindly.
For regular insurance, you are about to get another increase in premiums. It is going to be bad, and a family of five could be paying $15,000 a year or more, and that is with a big deductible. What I am saying is I get to hear about it every day, and the numbers don’t add up. The insurance companies will eat it this year, maybe next, but it is about to get even uglier. Nobody is fixing the core issue, which is that the cost of services is too high. We need to start at the service level and either put limits in place or open up competition. I don’t have the answer. I just see the issue. Of course, I am thinking of ways to monetize the info other than just buying a domain name. For you, just a warning.
